According to International Society for the Study of Vascular Anomalies classification 2018, &#x201c;hemangioma&#x201d; should be classified as either vascular tumor or vascular malformation (VM). So-called &#x201c;cavernous hemangioma&#x201d; is categorized as VM. VM rarely involves the mucous membranes of the sinonasal cavity and typically arises unilaterally from the sinonasal cavity. Bilateral VM of the maxillary sinus is extremely rare. To the best of our knowledge, there is no previous report of bilateral VM of the maxillary sinus. Here, we describe the surgical treatment of bilateral cavernous hemangiomas of the maxillary sinus. These tumors were successfully resected by endoscopic modified medial maxillectomy (EMMM) after embolization. Endoscopic sinus surgery, particularly EMMM, produces access to the bilateral maxillary sinus and can prevent several complications.

Surgical Management of Bilateral Venous Malformation (Cavernous Hemangiomas) of the Maxillary Sinus

We present a case of spontaneous cervical, retropharyngeal, and mediastinal emphysema occurring in a previously healthy young male, which is the first described case in Saudi Arabia. The patient was admitted to the ward for observation, monitoring of vital signs, analgesia, and prophylactic antibiotics. The patient was kept under observation for 8 days. During that time, neck pain improved gradually with no episodes of oxygen desaturation or vital sign deterioration. The patient was later discharged with very mild persistent pain. Two weeks after discharge, patient was seen in outpatient clinic and was free of symptoms. Spontaneous subcutaneous emphysema remains a rare presentation encountered in emergency department. The investigations and treatment required for such patients has no consensus between authors. Although most of the reported cases described an uncomplicated course, there is a need for clear guidelines on management protocol.

A Young Healthy Male with Spontaneous Subcutaneous Emphysema Occurring in Neck, Retropharyngeal and Mediastinal Spaces

Background. A cutaneous horn is a common clinical entity which usually presents as a cutaneous lesion. Because of its subtle nature, patients usually tend to present late unless the lesion is big or complications develop. Because of its resemblance to animal horn, it has been given the term &#x201c;horn.&#x201d; Cutaneous horn seems to have a remarkable history. Though cutaneous horn is benign most of the times, chances of malignancy (20&#x2013;25&#x0025;) should be kept in mind. Old age, giant cutaneous horn carries more chances of transformation into malignancy like in our case. Thus, early diagnosis and treatment is required in all cases. Case Presentation. We report a case of a 74-year-old farmer with a cutaneous projection measuring &#x223c;8&#x2009;&#xd7;&#x2009;5&#x2009;&#xd7;&#x2009;3&#x2009;cm3 over the medial surface of the right pinna for 1 year. It started as a small projection which was progressively enlarging. The primary reason behind him presenting to us was cosmetic reason since it resembled an animal horn. The projection was not associated with pain or similar lesions anywhere else in body. Understanding the malignancy risks and the cosmetic benefits, he was planned for excision biopsy of the horn. He had no systemic signs of malignancy. Histopathological reports were consistent with malignancy. Conclusions. Cutaneous horns are usually benign lesions and mostly found in the head and neck region. Because of the chances of malignancy, cutaneous horns should undergo surgical removal and biopsy for early and definitive diagnosis and management.

Verrucous Carcinoma in a Giant Cutaneous Horn: A Case Report and Literature Review

Transoral laryngeal procedures are considered minimally invasive but may trigger important complications such as severe bradycardia and even asystole mediated by vagal reflex. The literature on this subject is rare. This article aims to review the literature, explain associated mechanisms, establish prophylactic strategies, and highlight the importance of intraoperative safety protocols.
